Are you troubled by stress, anxiety or depression?  Feeling lonely, isolated and vulnerable?  The Community Connectors Project can put you in touch with a Community Care Navigator to support you to access non-medical activities and services.

The project aims to reduce the number of GP appointments being taken up by people that don’t require medical treatment.

Community support and services have been shown to improve the wellbeing of many people and we will help you to access the right ones for you.

We accept referrals from GP’s, health practitioners and social workers. We also accept self-referrals from individuals or their family.

Community Care Navigators are fantastic people that generously volunteer and will spend time socialising, supporting and delivering wellbeing back into your life. If you would like to take part in this vital project as a volunteer Navigator you will be welcomed with open arms!
